Health and Immunization Status Requirements
Health insurance | $400 |
Students who are selected for the clinical year are required to submit a Status of Health and Immunization form signed by their physician. As part of this requirement, they are asked to provide proof of immunity or vaccination for Rubella, Hepatitis B, and Varicella Zoster. Students must have health insurance during their hospital phase and are required to provide proof of it prior to hospital phase. The UWF Health Center uwf.edu/healthcenter provides immunizations, lab work, and physicals. The most current price list is on the website.
