To be eligible for services, the student must provide appropriate documentation from a licensed or certified professional with expertise in the specific disability. Students should be sure that:
*IEP's, 504 Plans, and Doctor's prescription notepads are not acceptable but can be used for supplemental information only.
A student is responsible for providing documentation that supports his/her request for academic adjustments. It is important that the documentation clearly establishes that the student has a disability as defined by federal regulations and that the documentation clearly supports the individual's request for accommodations. The following documentation guidelines are provided in the interest of ensuring that documentation demonstrates a significant impact on a major life activity and supports the individual's request for accommodations.
