The application for admission is required of all international exchange students who will be coming to UWF on a J-1 visa. This includes all students from UWF exchange partner institutions. No application fee is required. Fill out the application completely and return it with your Financial certification (bank statement or letter), Immunization form, Medical history form, and the Meningitis-hepatitis form to:

Exchange applications are due by July 1 for Fall exchange and by November 15 for Spring exchange.
We recommend that you submit a copy of your passport with your application, so that your demographic information will be entered correctly on all of your forms.
Sample DS-2019 and J-1 VISA
Once you have been admitted to the University of West Florida and have completed and returned all health and immunization forms, along with evidence of financial support, we can issue an DS-2019 for you.
Provide our office with an address in your home country where we can send the DS-2019 via Federal Express. The charges for Federal Express delivery will be added to your student account. The fees for shipping typically range from $40-75. We cannot ship the I-20 (DS-2019 for the J-1 page) using any other method unless Federal Express service is unavailable in your country.
