International students who are coming to UWF on F-1 visas and are seeking academic degrees are required to complete an application for admission. Fill out the application completely and return it with your Financial certification (bank statement or letter), Physician’s evaluation, Immunization form, Medical history form, and the Meningitis-hepatitis form to:

We recommend that you submit a copy of your passport with your application, so that your demographic information will be entered correctly on all of your forms.
Sample I-20 and F-1 Visa
Once you have been admitted to the University of West Florida and have completed and returned all health and immunization forms, along with evidence of financial support, we can issue an I-20 for you.
Provide our office with an address in your home country where we can send the I-20 via Federal Express. The charges for Federal Express delivery will be added to your student account. The fees for shipping typically range from $40-75. We cannot ship the I-20 (DS-2019 for the J-1 page) using any other method unless Federal Express service is unavailable in your country.
Transfer Students
If you are transferring to UWF from a college or university in the United States, in addition to being admitted to UWF, you also will have to be transferred into UWF's SEVIS system from the school that previously issued your I-20. This process requries that the international student advisor at your previous school must complete and sign the Transfer Form.
The international student advisor also will have to transfer your record out of your previous school's SEVIS system and into UWF's SEVIS system.
Once this has been achieved, we will be able to issue an I-20 based on your transfer status.
Note that these procedures must be followed whether you are in a degree program at another institution or participating in intensive English classes.
