The University of West Florida, in compliance with Florida Statute (1006.69) and Florida Administrative Code (6C-6.001-5), requires as a prerequisite to matriculation or registration:

For both rubeola and rubella, acceptable documentation as above must be from a health care provider such as your physician, clinic, health department records, military immunization records, or from your high school or college/university records, etc.
Effective July 1, 2008, all new UWF undergraduate and graduate students must provide documentation of vaccinations against meningococcal meningitis and hepatitis B or provide a signed waiver for each declined vaccination.
