Credit Hours Used Form

Students must submit the correct portion of the Credit Hours Used (CHU) form to this office within 10 days after registration. Failure to return the form will result in the forfeiture of your out-of-state waiver and disqualifying you from applying the waiver in the future. If you can not submit it directly online, please download the form and mail it to this office. Students who register for less than the number of hours awarded must send a copy of the registrar's receipt, or other proof of enrollment, to the Florida-China Linkage Institute office, so that we may award the unused hours to another student. Failure to do so may result in cancellation of the exemption.
