• is typed on the official letterhead of a qualified professional, institution, and or agency making the diagnosis,
• includes professional's name, title, dates of treatment, signature, license/certification number, address, and phone number
• lists the instruments used and standard scores received,
• provides a diagnosis that is clearly stated, explained, and specific,
• explains functional limitations as it pertains to academic setting,
• provides a rationale for services (relationship between diagnosis and accommodations),
• provides recommendations for effective and reasonable accommodations

A student is responsible for providing documentation that supports his/her request for academic adjustments. It is important that the documentation clearly establishes that the student has a disability as defined by federal regulations and that the documentation clearly supports the individual's request for accommodations.
